Case Manager - $50k - $60k

The Case Manager works with the case management and program team to offer case management and personal support to women living in Calvary’s transitional housing programs. The staff team works from a strengths-based model, offering resources to support women in achieving their goals for health, financial stability and independent living. The Case Manager works primarily on-site at the housing programs, but also attends off-site meetings and appointments with residents.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Work with a caseload of 15 women; developing and reaching the goals outlined in personalized case plans

  • Engage women in on-site services including therapy, recovery meetings, medical care, education and employment services

  • Make referrals for community-based services; strengthen relations with other service providers in the city

  • Complete intake process for new residents

  • Maintain current and accurate hard-copy and electronic case files

  • Work one evening shift each week; monthly emergency on-call shift
